You can call or E-mail us to set up a new patient appointment (usually 30 minute visit, longer if you have a complicated medical history). New patient information form can be filled out ahead of time and brought to the office at time of your visit, or  we can provide one for you when you come in.

 

You may wish to have medical records tranferred to us prior to your initial visit.  You can mail or fax a medical record transfer form to where your records are kept currently.
